Summary
The PERSIST project is a novel European collaboration aiming to improve the QoL of cancer survivors through patient-centred routine follow-up with the support of artificial intelligence (patient trajectories) and big data aggregating clinical data and real-world data. The approach includes harmonization and analysis of electronic health records (EHRs) in order to discover disease trajectories, and collection and integration of real-world data, alongside clinical data, into clinical routine. The PERSIST technologies are developed through a co-creation process with 160 cancer survivors (i.e., 80 breast and 80 colon cancer survivors) and their healthcare professionals from Belgium, Latvia, Slovenia, and Spain.
HosmartAI project will incorporate 8 large-scale pilots that will involve 3000 patients, 300 healthcare professionals, 600 stakeholders including healthcare managers in 5 different European regions (Belgium, Italy, Greece, Slovenia and Spain). Targeting differentiated medical aspects and manifestations such as Cancer, Gastrointestinal disorders, Cardiovascular diseases, Thoracic Disorders, Neurological diseases, Elderly Care and Neuropsychological Rehabilitation, Fetal Growth Restriction and Prematurity. HosmartAI will guarantee the integration of Digital and Robot technologies in new Healthcare environments and the possibility to analyse their benefits by providing a Platform where digital health care tool providers will be able to design and develop AI solutions as well as a space for the representation and deployment of AI solutions.
